SPORTS REGISTRATION
The Windsor Locks High School Athletic Department has partnered with FinalForms, an online forms and data management service, for sports registrations. FinalForms allows you to complete and sign athletic participation forms for your students. FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice. FinalForms also pre-populates information wherever possible, for each of your students, saving you time. You may review your data at any time to verify it is current.
You will be required to sign your forms once per year and after any update.

and follow the prompts to create your account, create your students and sign your forms. Parents MUST initiate the registration process. This is for high school students ONLY.
Click on the Parent Playbook and follow the step-by-step directions for registering with FinalForms. The process should take approximately 10 minutes to complete. Please provide the most accurate information possible.
Once a parent has completed the registration and signed ALL forms, the student will receive an email from FinalForms prompting them to sign the athlete forms.
Be sure to note that any student wishing to participate in a sport at WLHS MUST have a current physical on file with the nurse’s office. You will see there is an option to upload a physical on FinalForms. This is not required. Hard copies can still be brought to the nurse’s office.

FinalForms is now the only platform we will be using for athletics registration moving forward.
NO STUDENT WILL BE ALLOWED TO PARTICIPATE UNTIL THE ONLINE REGISTRATION HAS BEEN COMPLETED BY BOTH PARENT AND STUDENT AND A CURRENT PHYSICAL IS ON FILE WITH THE NURSE.
